SWEEPSTAKES OFFICIAL RULES
NO PURCHASE OR PAYMENT OR SURVEY PARTICIPATION
NECESSARY TO ENTER OR WIN. A PURCHASE OR SURVEY PARTICIPATION WILL NOT IMPROVE ONE’S
CHANCES OF WINNING. VOID WHERE PROHIBITED.
1.
ELIGIBILITY:
This Sweepstakes is offered to legal residents of the U.S. who are age eighteen
(18) or older as of the first day of each “Sweepstakes Period” listed on the “SWEEPSTAKES
ENTRY SCHEDULE” below, and who are physically
located within the United States (hereafter “Territory”) and who are not
employees of Dollar General Corporation (“Sponsor”), Administrator, or any of its
or their parent companies, subsidiaries, affiliates, assigns, advertising, or research
and promotion agencies. Members of the immediate family of persons above (spouses,
children, siblings, in-laws and members of the same household [whether related or
not]) are not eligible. Void in Rhode Island, Canada, Puerto Rico, Guam, U.S. Virgin
Islands and any other U.S. possession or territory and where prohibited or restricted
by law. Promotion is subject to all federal, state, provincial and local laws and
regulations.
2.
ENTRY:
Sweepstakes occurs monthly, according to the Dollar General Sweepstakes Entry
Schedule chart below; beginning February 2, 2013 at 12:00:01 a.m. Eastern Time (ET)
and ending February 1, 2014 at 11:59:59 p.m. Eastern Time (ET). Participants may
enter during each listed “Sweepstakes Period”.
There are two ways to enter:
a.
Eligible
customers of participating Dollar General® retail locations will receive a cash
register receipt inviting them to call a toll-free telephone number (1-866-214-5750)
or visit the Internet address (http://www.DollarGeneralSurvey.com) to participate in a Survey and/or enter
the Sweepstakes. Customers who receive an official Survey Code can complete a customer
satisfaction survey online during a Sweepstakes Period (a "period" being
defined as between 12:00:01 a.m. (ET) on the first day of a monthly sweepstakes
period and 11:59:59 p.m. (ET) on the last day of that monthly sweepstakes period)
in order to enter for that period’s drawing.
b.
Alternatively,
any eligible person may enter the drawing by hand printing their full name, street
address (no P.O. Boxes), city, state, zip code, home telephone number, e-mail address
and age on a 3˝” X 5” postcard and mailing it, first-class U.S. postage prepaid,
inside a standard #10 size business envelope with a handwritten address to:
Dollar General Customer Satisfaction Survey Sweepstakes, P.O. Box 690, Georgetown,
CT 06829-0690. All mail-in entries must
be postmarked on or before the last day of a sweepstakes period and received by
the 7th day of the immediately following period for entry into that period’s drawing.
If the last day of a period and/or the 7th day of the immediately following period
falls on a Sunday or a government Holiday, the entry may be postmarked or received
(as appropriate) on the immediately following business day. Entries must be submitted
individually in each envelope with the entry address handwritten thereon. No mechanical
reproductions or facsimiles will be accepted. Any alterations or forged entries
will void the entry. Any entries not received
in envelopes will be disqualified. Any professionally or mechanically printed entry
forms other than those hand written on a plain 3˝” X 5” postcard will be disqualified.
Illegible, incomplete, mechanically
reproduced, multiple or automated entries, and entries by ineligible individuals
will be disqualified. Lost, late, misdirected, mutilated or postage-due entries
will not be considered. Each postcard-envelope
entry must be mailed
separately
in a #10 size business envelope with a handwritten address.
Limit only one entry per person or household each monthly Sweepstakes Period, regardless
of method of entry. Entries received for a given period’s drawing will not be retained
for subsequent periods’ drawings.

Only complete entries are eligible. In case of dispute as to identity of an online
entrant, entry will be declared made by the authorized account holder of the E-mail
address used to enter the Sweepstakes at time of entry. "Authorized Account
Holder" is defined as the natural person who is assigned an E-mail address
by an Internet access provider, online service provider, or other organization (e.g.,
business, educational, institution, etc.) responsible for assigning E-mail addresses
or the domain associated with the submitted E-mail address. Any other attempted
form of entry is prohibited; automatic, programmed, robotic or similar means of
entry are not permitted. Sponsor, its advertising, promotion and research agencies
are not responsible for lost, late, damaged, incomplete, illegible, misdirected
or postage-due entries/mail or for technical errors of any kind including computer/information
network failures, hardware, software, telephone or other communications malfunctions
of any nature, errors or failures of any kind, lost or unavailable telephone or
network connections, Website, Internet, or ISP availability, unauthorized human
intervention, traffic congestion, incomplete or inaccurate capture of entry information
(regardless of cause) or failed, incomplete, garbled, jumbled or delayed computer
transmissions which may limit a participant’s ability to take part in the Sweepstakes.
All entries become the sole property of the Sponsor and will not be acknowledged
or returned.
CAUTION: ANY ATTEMPT BY AN
ENTRANT TO DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATIONS OF THE SWEEPSTAKES
IS A VIOLATION OF CRIMINAL AND/OR CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, THE
SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEY’S FEES)
FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW, INCLUDING CRIMINAL
PROSECUTION AND DISQUALIFY SUCH ENTRANT FROM THE SWEEPSTAKES.
3.
DRAWING:
Winners will be selected in a series of three (3) monthly random drawings with three
(3) drawings (and three winners) per Dollar General monthly Sweepstakes
Period - that is, three drawings per sweepstakes period from February 2, 2013 to
January 31, 2014. Each period’s drawing will consist of all eligible entries received
for that “Sweepstakes Period” (see Section 2 "ENTRY", above). All drawings
will be conducted by PHD Marketing Services LLC, an independent judging organization,
on or about the fifteenth day of the immediately following sweepstakes period. Odds of winning depend on the total number
of eligible entries received. The Administrator’s and/or Sponsor’s decisions
will be final and binding in all matters relating to the Sweepstakes. Sponsor reserves
the right, in its sole discretion, to cancel, terminate, modify, or suspend this
Sweepstakes in whole or in part for any reason, including, without limitation, to
change the sweepstakes Rules or prizes in any jurisdiction or if causes beyond its
control corrupt or affect the administration, security, fairness or proper conduct
of the Sweepstakes. In such case, Sponsor will select the winner from all eligible
entries received prior to and/or after (if appropriate) the action taken by Sponsor.
By entering Sweepstakes, entrants agree to be bound by these Rules and the decisions
of the judges.
4.
PRIZES: Three (3) Grand Prizes for each
sweepstakes period’s drawing:
A check in the amount of $1,000 will be presented to each Grand Prize winner. Retail
value of Grand Prize is $1,000. The prize awarded in connection with the Sweepstakes
is nontransferable and non-endorsable. The prize (or any portion thereof) cannot
be sold or transferred. All prizes claimed will be awarded. No prize substitutions
allowed, except at Sponsor’s sole discretion in the event of prize unavailability
for any reason, and then only for a prize of equal or greater value. All federal,
state, provincial and local laws and regulations apply.
5.
NOTIFICATION:
The three (3) Grand Prize winners selected during each sweepstakes period will each
be notified by phone, e-mail [if provided] and/or overnight courier. When initially
notified by phone, entrant must respond within forty eight hours (48 hrs.) that
are interested in receiving the prize, or the next alternate winner will be contacted.
Winner will be required to sign and return
an Affidavit of Eligibility and a Liability and Publicity Release (where legal)
and a W-9 Tax identification form within 10 days of notification. If winner cannot
be contacted, declines to accept the prize, or doesn’t respond to contact attempts,
or if prize is returned as undeliverable, or in the event of noncompliance, as determined
in the discretion of Administrator and/or Sponsor with these Official Rules, the
prize will be forfeited and an alternate winner will be selected. Upon forfeiture,
no compensation will be given. Prize will be awarded to winner or an alternate winner
under all circumstances.
6.
CONDITIONS:
All taxes are the sole responsibility of the winner accepting the prize. An IRS
1099MISC Federal tax form will be issued to the winner for the value of the prize.
Winner will be required to complete and sign a Federal W-9 tax identification form
before receiving their prize. Participation in Sweepstakes and acceptance of prize
constitutes winner’s permission to use his/her name, address (city and state only),
likeness and any statements made by winner regarding the Sweepstakes or Sponsor
for advertising and promotional purposes without additional compensation, except
where prohibited by law. By acceptance and use
of prize, winner agrees to hold harmless and indemnify Sponsor and its parent organization,
affiliates, governors, officers, directors, agents, employees, subsidiaries and
service agencies or independent contractors against any and all liability, damages
or causes of action (however named or described), with respect to or arising out
of entrant’s participation in the Sweepstakes, or the receipt or use/misuse of the
prizes awarded herein. These parties,
their respective affiliates and subsidiaries, advertising/promotional agencies,
and each of their directors, officers, employees, agents, successors and assigns
have no liability or responsibility for any personal injuries, losses or damages
or claims of any kind resulting from participation in the Sweepstakes, acceptance,
possession or use/misuse of any prize. Sponsor assumes no responsibility for any
error, omission, interruption, deletion, defect, delay in operation or transmission,
theft or destruction. The Sponsor is not responsible for printing or typographical
errors in any Sweepstakes related material. No more than the advertised number of
prizes will be awarded. Entrants who do not comply with the Official Rules or attempt
to interfere with this Sweepstakes in any way shall be disqualified. There is no
purchase or sales presentation necessary to participate. These Official Rules are
intended solely for the personal use of an individual participant and may be reproduced
only
once for such personal use; ANY
FURTHER REPRODUCTION OF THESE OFFICIAL RULES IS PROHIBITED.
7.
ADDITIONAL TERMS:
Sponsor reserves the right, in its sole discretion, to disqualify any individual
it believes, in its sole discretion, to be tampering with the entry process or the
operation of the Sweepstakes or Web site. For details regarding collection of information
from users of the Web site (including entrants), please consult the privacy policy
on the Website located at
http://www.dollargeneral.com/helpdesk/index.jsp?display=safety&subdisplay=privacy. Additionally, Sponsor reserves the right
to prosecute any fraudulent activities to the full extent of the law.
8.
WINNERS LIST:
For the first initial, last name and city of residence of the period’s prize winner,
send a self-addressed stamped envelope, postmarked no later than the 15th
of the sweepstakes period immediately following to: 2013-2014 Dollar General Customer
Satisfaction Survey Sweepstakes [Specify Sweepstakes
Period] Winners List, P.O. Box 746, Georgetown, CT 06829-0746. Residents
of VT may omit return postage.
